A quality-improvement study on the discharge process, where medication errors are most likely to reach the patient. I helped examine how reconciliation was being done at discharge, where the gaps appeared between the chart and what patients actually went home with, and which changes to the workflow closed those gaps most reliably.

It walks through the barriers — language, cost, stigma, displacement trauma — and the interventions that have actually shown results, such as culturally matched community health workers and screening built into resettlement.
